CANCELLATION POLICY: As a courtesy, please give notice by 4pm the day prior to your appointment. If an appointment is canceled within 4 hours of your scheduled time, a $30 non-cancellation fee will be charged to your account. This fee is not billable to insurance and must be paid prior to your next visit.
NEW PATIENTS: Please arrive 15 minutes prior to your scheduled appointment to allow time for administrative tasks required for your first visit, including creation of patient record, insurance verification and completion of clinical forms.
After you choose a date & time, you will create an account with your details and complete the booking.
Your email address and password will then allow you to review, change and add appointments.
